Step 1 :- First of all you will have to visit CEO Bihar's Website by Clicking Here and go on "Forms" Tab showing in left side of the website page. 

Step 2 :- When you will click on this tab, there will be a Page like this. 

Step 3 :- You have to Select Form 6 and After filling up the form and attach required documents by the CEO, Bihar.

After Filling up the Form 6 for new Voter ID Card Registration, this form is to be submitted to the Electoral Registration Officer of your Constituency along with an Application on Blank Paper.
